InsideOut Institute and the General Practice Mental Health Standards Collaboration (GPMHSC) have developed new eating disorder care plan templates. Co-designed and reviewed by GPs, Practice Managers, Allied Health & Lived experience consultants.
Available for download below including formatted versions for use in your software.
InsideOut Institute has developed a new set of care plan templates specifically for patients living with an eating disorder.
These include:
- Mental Health Care Plans (for eating disorders)
- GP Management plans
Formatted templates will auto fill and auto prompt in your software and save GP time.
Be careful! Do not open these files before you import to your software, as formatting will be lost.
